Set up SOAP notes

SOAP notes are treatment notes your team writes for an appointment, split into Subjective, Objective, Assessment and Plan. Templates give your team a ready-made note to start from, and Note Settings decide when and how notes are signed.

Create a SOAP note template

  1. 1

Go to Settings > Clients > SOAP Notes. Your templates are listed under Templates, with how many images each one has.

  2. 2

    Click Add SOAP Note Template.

  3. 3

Enter a Name, like Massage therapy.

  4. 4

Write the Default note. It starts with the four SOAP headings, so add prompts under each one for what your team should record. Use the toolbar for bold, lists, links, emojis and special characters.

  5. 5

Optionally, add images under Attachments, such as a body chart. They're added to every note created from the template. Each image can be up to 5 MB.

  6. 6

    Save the template.

To change a template, click it in the list. Make your changes, then click the more options (three dots) button at the top and select Save. To delete it, use Delete in the same menu, or the more options button next to it in the list. Deleting a template doesn't change notes already written from it.

Choose your note settings

  1. 1

On the SOAP Notes page, click Note Settings.

  2. 2

Turn on the options you need, choose a Signature type and click Save.

    • Require team member signature - unsigned notes show Signature required until they're signed.
    • Require signature to lock - a note can only be locked once it's signed.
    • Signature type - team members sign with a Typed name, a Drawn signature, Either one, or Both.
