Create a form template
A form template is the set of questions you want answered. Templates start out disabled, so you can build and preview them before anyone sees them.
Create the template

Go to Settings > Intake Forms and click Add Intake Form Template. You can also click Manage templates from Clients > Forms.

Enter a Name and an optional Description. Clients see both when they fill out the form.

Click Create.
Choose the form settings
In the Details section, answer three questions:
- Who fills out this form? - clients, staff members, or anyone with the link.
- How often? - every time the client books, or only once for each client.
- Does this form require review? - No review needed, Review required by chosen staff members, or Review & signature required.
Add form fields

Open the Form Fields section and choose a field type: First/Last Name, Email, Phone Number, Date picker, Short Answer, Long Answer, Dropdown, Multiple Choice, Choose One or Toggle.

Enter the Field label. For Dropdown, Multiple Choice and Choose One, add the options clients can pick.

Turn on Require this field if clients must answer it, then click Add field.

Repeat for each question. Click Reorder to change the order, or use a field's more options (three dots) button to edit or remove it.
Preview and enable the form

Open Preview to see the form the way it will be filled out.

Under Advanced, add an optional Custom confirmation message shown after the form is submitted.

To save changes, click the more options (three dots) button at the top and select Save. The same menu lets you Duplicate or Delete the template.

When the form is ready, click Enable next to the form name and click Confirm.
An enabled client form only applies to appointments booked after you enable it.
Request a form automatically for a service
Go to Catalog > Services, open a service and pick the form under Client's intake form. The form is then added automatically to every new appointment for that service.
